Steps for ordering paper Application Form

1. Fill Contact Information
2. Pay through Credit Card
3. Save Receipt of payment on your computer
4. After this you shall receive the paper Application Form through courier. The instructions for filling the same shall be given with the paper Application Form.
5. Dispatch the filled out Application Form to the designated address.

You will need the following for collecting paper Application Form:-

1. Demand Draft of Rs. 1000 in favor of “Secretary, Association of Private Dental & Medical Colleges of Uttar Pradesh” payable at Bareilly.

Steps for collecting paper Application Form

1. Prepare Demand Draft of Rs. 1000 in favor of “Secretary, Association of Private Dental & Medical Colleges of Uttar Pradesh” payable at Bareilly.
2. Approach Authorized Centers with Demand Draft and collect paper Application Form.
3. The instructions for filling the Form shall be given with the paper Application Form.
4. Dispatch the filled out Application Form to the designated address.
